Martin Health is seeking a full time Clinical Pharmacy Specialist who under the supervision of the Clinical Pharmacy Manager and the Pharmacy Operations Manager will help oversee clinical pharmacy activities for the health system.
As the Clinical Pharmacy Specialist, you will develop monthly agendas with assistance from the management team, prepare monthly meeting materials for the P&T staff, prepare detailed drug monographs for formulary addition requests, conduct quarterly Medication Use Evaluations (MUE) and present at P&T meetings, coordinate any follow-up needed from the P&T meeting, prepare any memos to be distributed from the P&T Executive Committee and review formulary on an annual basis (with formulary sub-committee) to make recommendations for positive change.
Ideal candidates will have a demonstrated knowledge and skills in clinical activities including but not limited to pharmacokinetic dosing, TPN dosing, medication use evaluations, adverse drug reaction programs, antibiotic utilization, formulary monographs. Additionally, a Doctor of Pharmacy Degree, an active Florida Pharmacist license, five years of acute care clinical pharmacy hospital experience (or PGY1 Residency and three years of clinical pharmacy hospital experience), in addition to strong organizational, communication and motivational skills are required for consideration.
