This Social Services Counselor is responsible for the Social Work serving Shady Oaks Village at Northeast Florida State Hospital, a treatment facility for persons with mental health issues. Shady Oaks is a certified unit under the auspices of the Centers for Medicare and Medicaid (CMS). This individual provides delivery of therapeutic activities and clinical documentation; observes and provides feedback to the Recovery Teams, provide therapeutic interventions and implement on-going actions necessary for discharge of Shady Oaks Persons served that are placed on discharge status. This position requires a Master’s degree in Social Work from an accredited college or university and three years of professional experience in developmental disabilities, special education, mental health, counseling, guidance, social work, health or rehabilitative services. You will be required to work a rotation every 4th Saturday and, as required by all direct care staff, may be called upon to assist persons in treatment with community outings focused on resolving discharge barriers, selecting community discharge sites or travel to an actual discharge destination. A valid driver’s license is therefore required for this position.
